Return Merchandise Authorization Request

At Glaz Chips™ Fire Glass our main objective is to bring you the best customer service and satisfaction with future and/or previous purchases. If for any reason your current purchase of Glaz Chips™ Fire Glass was unsatisfactory or would like to return the product, please fill out the form below within 30 days of receipt. After form has been filled out to its entirety you will be contacted via e-mail with instructions on how to return your merchandise. Please keep in mind that this process may take 5 -7 business days. A confirmation will be sent to you for your personal records once your form is filled out and submitted.

Terms and Conditions

Upon submitting this form you will receive a Reference ID No. along with shipping instructions. Glaz Chips™ Fire Glass will not however receive partial product. The customer must ship back all packaging content(s). No readjustments will be granted.

1. By submitting this form does not automatically guarantee a refund.
2. Returns will not be processed / inspected without an RMA.
3. RMA's will be issued by one of our technical support staff.
4. The RMA being submitted will be valid from 30 days from submission.
5. Once your purchase has been received testing will commence.

Product Testing Program

DEFECTIVE: After inspection and the item is concluded to be defective, we will replace your order at our discretion along with incoming/outgoing shipping costs.
NON-DEFECTIVE: Based upon thorough inspection and if item is to be concluded as non-defective, it will be returned to the customer - no credit will be given. Shipping costs, freight in and out are at the customer's expense.
GCFG ERROR: In the event that the product the customer received was a mistake on Glaz Chips™ Fire Glass part, a RMA and Return Services Label will be issued. When products have been received, a credit will be issued along with outgoing shipping/freight costs on the original invoice.
CUSTOMER ERROR: RMA will be issued if requested within 30 days of original purchase. If the customer placed an incorrect order, the customer will be responsible for shipping, freight in and out costs. Once products have been received, inspected, and weighed a credit may be issued to the customer. A restock fee of 20% will be added.
EXCESS INVENTORY: If an RMA is requested due to customer excessive inventory ( products in original packages only ) Glaz Chips™ may issue the RMA. The customer is responsible for the return shipping. Once products / items have been received, inspected, and weighed Glaz Chips™ will issue credit for the customer. A restock fee of 20% will be added.
